Our Office Policies

We look forward to seeing you on your scheduled appointment date. If you are a new patient, information is being sent to you in an effort to streamline the check-in process and answer any questions you may have regarding your visit. We will be contacting you a few days prior to your appointment to confirm the appointment date and time, pre-register your demographic information and remind you to have a driver available to take you home after the appointment if needed. When you check-in for your appointment, we will need to copy your insurance card(s) in order to bill your insurance company.

Billing

The Mid-Michigan Heart & Vascular Center accepts cash and personal checks as well as the following credit cards:

  • Personal Check

We also accept these payment options :

All co-payments and are due at the time of service.  We accept cash, check, and Visa, MasterCard, American Express and Discover credit cards.

Appointments/Cancellations

With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators.

We understand that your schedule may change and that emergencies arise. If you are unable to keep your appointment, we ask that you kindly provide us with 24-hours notice. Please extend us this courtesy so that other patients may be seen.  Our office can be reached by calling 989-249-6432.

Emergencies

Our office provides on-call service for emergencies. Call 989-249-6432 so that the physician on call can be contacted.

If you have an emergency, please dial 911.  After office hours, our answering service will take your message and immediately contact our physician. Our telephone is answered 24 hours a day, 7 days a week, by calling 989-249-6432.
